Step 1: Fill Out Repair Form
Download the NSK Dental Repair Form below, fill in the information and ship it with your product.
A purchase order number is required to process your repair. Please contact your distributor for a purchase order number.

Step 2: Print Shipping Label to Ship Product
Please fill out the form below and click submit to generate a label in .PNG format to print and place it on your package for shipping. The label will also be sent to your email for your records.

Step 3: Include the invoice for Warranty claims
If the repair is for a warranty claim, you must include the invoice.
